The Colorado Rural Health Center (CRHC) recognized Foresight Family Physicians as this month’s featured member

Denver, Colo – Foresight Family Physicians, located in Grand Junction, provides quality, caring, patient-centered healthcare to residents in Grand Junction and surrounding Mesa County. Foresight Family Physicians joined Colorado Rural Health Center’s membership in 2015.

Grand Junction is the largest city on Colorado’s Western Slope, and serves as a hub for transportation and shipping between Denver and Salt Lake City. The name Grand Junction relates to the junction of the Colorado River and the Gunnison River. The area remains one of Colorado’s largest food producers, including peaches, pears, cherries and corn – and is known as Colorado’s wine country, for the more than 20 wineries located in the region.

Foresight Family Physicians has adopted The Patient Centered Medical Home philosophy of health care delivery. Patients have an ongoing relationship with their personal physician who leads a team of individuals who take responsibility for the care of each patient.
