Listed below are the three levels of credentialing that CPR can provide for communities. All three levels include the candidate's current curriculum vitae, a list of at least 3 references, and a personal statement about the candidate.

Basic Verification Services	$35 / provider
Includes:
Candidate's current Curriculum Vitae List of 3 References Personal Statement License Verification Education Verification Board Certification Verification

Intermediate Verification Services**	$50 / provider
Includes:
All services provided in Basic Verification Services plus DEA Verification Medicare/Medicaid Sanctions State Medical Board Verification National Practitioner Databank Adverse Licensure Information Clinical Privileges Actions Professional Society Membership Actions (for physicians only)

Deluxe Verification Services**	$100 / provider
Includes:
All Services provided in Basic and Intermediate Verification Services plus Hospital Admitting Privileges Malpractice Insurance Coverage 5 Year Work History Credential Application covering JCAHO, NCQA & URAC Requirements Applicant Attestation Form
