Services for Providers

CPR offers a variety of services at no cost to providers. These services include:



Provider Interview Form

Providers interested in receiving detailed information regarding practice opportunities in rural and underserved communities in Colorado should first complete the Provider Interview Form (click below to download the appropriate Provider Interview Form in Word). Provider information will be kept confidential and will not be provided to third parties until provider approval has been received.

Community and Practice Profiles

Providers who are matched to an opportunity will receive detailed community and practice profiles. These profiles contain information on recreation, schools, community history, and county demographics. These profiles may also contain links to the local Chamber of Commerce or city/county websites. After reviewing the profiles, the provider should notify CPR of their level of interest in the opportunity. CPR will facilitate communication between the community and the provider.


3R Net

CPR knows that providers also look for opportunities in other states. The 3R Net website allows you to look for rural opportunities in states other than Colorado. Loan Repayment

The following loan repayment programs are available to providers practicing in Colorado:

  • National Health Service Corps
    This program is federally funded and administered. Loan repayment is available to primary care providers (including FP, IM, Peds, OB/GYN, FNP, PA, CNM, mental health and oral health) who commit to practice in a Health Professional Shortage Area (HPSA) for two years.
  • Colorado Health Professional Loan Repayment Program
    This program is federally funded and administered by the Colorado AHEC program. Loan repayment is available for primary care providers (including FP, IM, Peds, OB/GYN, FNP, PA, CNM, mental health and oral health who commit to practicing in a HPSA for a minimum of 2 years.
  • Colorado Rural Outreach Program (CROP)
    This program is administered by the Colorado Rural Health Center. Grant funds are available to physicians, physician assistants, and advanced practice nurses who practice in a rural area.
  • Loan Repayment for Dentists and Dental Hygienists
    This program is state funded and administered through the Colorado Department of Public Health and Environment. Loan repayment is available to dentists and hygienists who agree to serve medically underserved populations (does not need to be rural). Level of participation is determined by the number of underserved patients served per month.More information regarding loan repayment can be obtained by contacting Colorado Provider Recruitment at cpr@coruralhealth.org


Contract Assistance
