Developing Best Practices for the Care of Cancer Survivors: Learning from the Field

The number of individuals with a history of cancer presenting to primary care practices is increasing in number and is expected to continue to grow.   Many of them report unmet needs and gaps in care, especially those diagnosed as a child, adolescent or young adult.  This session will briefly review the scope of the problem and what we have learned with our work in Colorado, ultimately leading to an open discussion of strategies, best practices and available resources and guidelines to help support the needs of these individuals.
