Family Practice of Holyoke Recognized by Colorado Rural Health Center as this Month’s Featured Member

Denver, Colo – Family Practice of Holyoke is a certified rural health clinic located in Holyoke, Colorado.

Holyoke located in Phillips County – the Colorado prairie of northeastern Colorado – has a population just over 2,000 residents. Holyoke boasts a lively downtown with its rebuilt historic movie theatre and several historic buildings on the National Register of Historic Places. In addition to a deep appreciation for history, Holyoke offers many community events, recreational activities and fine restaurants.

Family Practice of Holyoke is vital to the community, providing a full range of experienced, compassionate health care services. The clinic is staffed by two full-time physicians, one part-time physician, and two full-time nurse practitioners. The clinic team is dedicated to providing the highest quality of health care services to residents of Holyoke and the surrounding area.

Sheila Robinson, the clinic manager, works extensively with Colorado Rural Health Center (CRHC) on a number of quality initiatives. For instance, the clinic participates in CRHC’s iCARE program for rural health clinics (RHCs). RHCs in the program submit a series of thirteen core diabetic data measures acquired from the National Center for Quality Assurance (NCQA). Aggregated data is shared and discussed during monthly project webinars.

The clinic has also completed Healthy Clinic Assessments (HCAs). The assessment is completed by a CRHC quality improvement specialist and allows the clinic team to review basic business operations and processes.
