About the Rural Connectivity Program
The Rural Connectivity Program (RCP) is a program funded by the Office of eHealth Innovation, Department of Health Care Policy and Financing and in partnership with the Colorado Community Managed Care Network (CCMCN).
The Rural Connectivity Program was implemented to increase access to health information technology (HIT) resources and analytics for Colorado’s Rural Health Clinics (RHCs) and Critical Access Hospitals (CAHs). The goal is to establish a sustainable model for rural connectivity, by connecting providers to Colorado’s Health Information Exchanges (HIE), supporting America’s rural and underserved populations with high-speed broadband.

The Rural Connectivity Program offers a multitude of benefits, making it a valuable opportunity for healthcare facilities.
Community Analytics Platform
The Community Analytics Platform (CAP) is a comprehensive dashboard containing crucial data on:
- Population health, including specifics on chronic diseases
- Outmigration
- ADT (Admission, Discharge, Transfer)
- Operational opportunities
- And much more!
By accessing the analytics provided through the CAP, providers are better supported with initiatives that address the following:
- Developing programs
- Understanding costs
- Refining workflows
- Population health strategies
This wealth of information empowers healthcare providers to make informed decisions and deliver higher-quality care.
Health Information Exchange
The RCP program facilitates free Health Information Exchange (HIE) connections with key organizations such as Contexture and Quality Health Network (QHN). This seamless connectivity enhances collaboration and data sharing, ultimately improving patient outcomes.
Data sources we collect from: CIVHC, LABs, HIEs, CDPHE, HCPF, 834 Medicaid patients, and demographics from participants EMR.
Rural Sustainability Program
An additional benefit the RCP presents is the potential for significant financial support through the Rural Sustainability payment.
While this payment is pending approval, it demonstrates a commitment to allocating resources for rural clinics and critical access hospitals. If approved, yearly appropriations of $20,000 will be made available to qualifying rural clinics and $100,000 to critical access hospitals.
To be eligible for this funding, facilities must fully participate in the Rural Connectivity Program.

Rural Connectivity User Group Meetings
CCMCN, in collaboration with CRHC, hosts Super User Group meetings on the 2nd Tuesday of every quarter to demonstrate what HIT is available to clinics, as a part of the Rural Connectivity Project, and how it can be insightful to providers and their organizations.
Participants will also have the opportunity to provide feedback on current reports, vocalize their organizational needs, and contribute to reporting enhancements.
