CRHC announces partnership with Telligen

In January, CRHC announced a partnership with Telligen to investigate innovations  to advance local efforts for better care at lower costs. These innovations included exploring the adaptation of a telephonic, evidence-based models, C-TraC to reduce 30-day readmissions. The successful first 2 kick-off trainings took place in Walsenberg and Holyoke. Additional participating facilities are Melissa Memorial, Rio Grande Hospital, Spanish Peaks Regional Health Center, and Conejos County Hospital.

C-TraC was developed by the University of Wisconsin, Madison Department of Medicine.
