The Rural Connectivity Program (RCP) bridges the gap between advanced data tools and rural healthcare delivery. Through the Community Analytics Platform (CAP), members gain access to 11 specialized reports, including the Admissions, Discharge, and Transfer (ADT) summary. This report provides real-time visibility into emergency department activity, inpatient transitions, and outpatient visits, enabling providers to strengthen care coordination, reduce patient outmigration, and improve the resilience of Colorado’s rural health systems.

Updated three times daily using data from the state’s HIE (Contexture), the ADT Summary Report delivers near real-time insights into patients seen across care settings. These frequent updates support faster decision-making, allowing care teams to respond promptly to patient needs and improve continuity of care.

Prioritize the Follow-up Gap

With nearly 47% of rural Coloradans struggling to get timely appointments, the ADT report allows your team to prioritize high-risk patients as well as frequent ED utilizers and provide timely outreach and follow up care to reduce unnecessary ED visits, lower costs, and improve care coordination.

 

Acknowledge Distance Barriers

A “non-emergent” ED visit often reflects the reality of rural travel distances. Use ADT data to identify patients who may need more flexible clinic hours, telehealth options, remote patient monitoring, or transportation support to help address geographic barriers and competing priorities.

 

Visualizing Trends Over Time

The ADT report includes messages from the past 90 days, helping you spot patterns over time. Use these trends to answer questions like:

  • Are certain zip codes or outlying areas showing higher rates of “non-emergent” ED use?
    • If patients from specific remote areas are consistently using the ED for primary care needs, it may indicate a need for targeted telehealth outreach or mobile clinic services in those zones.
  • Is there a pattern of “repeat” visits for chronic conditions like heart failure or diabetes?
    • By tracking these trends over 90 days, you can identify if a lack of specialized outpatient support or medication adherence is driving patients back to the ED for manageable symptoms.
  • Are ED visits spiking for respiratory issues?
    • With asthma hospitalizations doubling since 2020, trend data can help you assess whether local wildfire smoke or air quality is contributing to increased ED use.

How to Take Action

  • Daily Huddles (Care Teams/QI): Review refreshed ADT data throughout the day and use the “Diagnosis” field to identify members who may be delaying primary care due to cost or access.
  • 90-Day Trends (Leaders/Operations): Identify “frequent flyers” and cross-reference with social determinants data to strengthen community discharge pathways.
  • Best practice: timely follow-up. Research suggests follow-up schedule within 7 to 14 days of discharge can reduce the risk of complications.
