A Rural Health Clinic in Northeast Colorado is seeking a full-time Family Physician who is dedicated to providing the highest quality of care in Patient Centered Medical Home model, located in a quiet, country setting on the eastern plains!

A Rural Health Clinic is looking for a full-time Family Physician with an emphasis on team care. Daily activity consists of anything from DOT examinations, to typical URI’s, pain, ear aches, dermatology, and well-child visits. The practice is a NCQA Certification as a Level II Medical Home. The clinic is open Monday through Friday, and the new provider will work four days out of the week. Saturdays will be required once a month for four hours. The maximum call schedule is one day per week and one weekend per month at most. There will be ER coverage on call days. Pre-natal care is required for this position. The new provider will admit patients requiring hospitalization and will have rounds in the morning. There will be occasional travel to cover a satellite clinic about 28 miles away. This facility is not able to support a J-1 Visa Waiver.
The new provider will be joining a team of six physicians and one physician assistant. The patient population consists of people on Medicaid, Medicare and the uninsured. A sliding fee scale is offered for low-income patients. The clinic is located within the 12 bed admitting hospital. All services provided are consistent with a primary care hospital with the exception of obstetric services. The town itself boasts a low cost of living, an unemployment rate well below the state average, easy access to the Denver International Airport and urban amenities, and a 2-3 hour commute to the breathtaking Rocky Mountains!
