Jennifer Grant, QI Programs Manager

As the QI Programs Manager, Jennifer supports our rural clinics and hospitals in applying program management principles to enhance performance and provides management to our Quality Improvement Specialists. She provides assistance to our rural healthcare facilities through CRHC’s programs by providing tools, resources, technical assistance, and coaching to improve communication in transitions of care, reduce readmission rates, improve clinical and operational processes, support operations and infrastructure, and ensure compliance is being met for various state and federal regulations to assist our clinics and hospitals in remaining a viable and sustainable point of care.

Jennifer has a Bachelor of Science in Biology, Professional Advanced Certificate in Gerontology, CRHCP certification and is currently working on her CPHQ.  Prior to joining CRHC, Jennifer spent several years working on Quality Initiatives within the long-term care and clinic settings and spent over fifteen years as a Director of Assisted Living serving the low-income senior community. She has a genuine passion for advocating for and improving the quality of lives of others and hopes to use her experience within the long-term care setting to assist our rural communities.

Outside of work, Jennifer enjoys thunderstorms, bowling, throwing pottery, and swimming with the sharks, dolphins and stingrays while cruising.
