CRHC’s Annual Membership Survey

Requesting your Feedback

CRHC’s annual Membership Survey is now open, and we invite individuals at all CRHC member facilities, organizations and institutions to share their feedback! The survey includes questions on services, programs, communications, and CRHC as an organization.

CRHC will compile findings from this survey to present to the CRHC board of directors and for use internally to evaluate current CRHC programs and services. All data will be presented in aggregate form and individual responses will never be shared or disseminated. We encourage respondents to provide their contact information so that we can follow up if needed.
