Applicants are encouraged to apply 2-3 months prior to the start date of the course. This is because applications are accepted and reviewed on a monthly basis, so in the event your application is denied the first time, you can reapply the following month.

  • MUST BE a private or public organization in Colorado with their purpose the provision of emergency medical and trauma services in Colorado to be eligible. This grant program is intended to provide funding to urban and rural EMTS agencies for education and training of emergency medical and trauma service providers working in Colorado only. Individual persons cannot apply for CREATE. They must apply as the organization where they work or volunteer.

  • Multi-Agency Applicants:

    Applications are considered multi-agency when a request for training includes multiple agencies being reimbursed or covered by the agency applying for CREATE. Applications are considered single-agency if the only participants come from the applicant entity or if participants from outside entities are invited but would not be reimbursed or covered by the agency applying for CREATE. ***IF YOU HAVE A MULTI-AGENCY APPLICATION, be sure to send all participating agencies this 'Information Request Packet":
  • Create and/or Update Organizational Profile Information Form in ImageTrend (OATH)

    An Organizational Profile Information Form is required for all entities completing a CREATE grant and is a prerequisite to accessing the grant application. This is NOT the education program recognition gatekeeper process. The profile status must show "Updated" with a certification date during the current state fiscal year (July 1 - June 30).

    Log in/create account in CDPHE's ImageTrend portal.

    Reference this OATH instructional for guidance on creating or updating an organizational profile.

    Questions or concerns regarding OATH should be directed to CDPHE.

    If yes, upload the PDF output below (sent to your organization's profile gatekeeper).

    If no, your organization's profile gatekeeper must update the profile information form in OATH and provide the output PDF.
