Rural Connectivity Program – A Year in Review
Tis the season! It’s a time for togetherness, reflection, and celebration. As 2025 comes to a close, we at the Rural Connectivity Program want to celebrate the hard work and meaningful progress made toward improving rural health quality and access. Through collaboration, innovation, and dedication, we’ve strengthened data-driven decision-making, enhanced workflows, and supported Colorado’s rural health facilities in delivering better care. Here’s what we accomplished this year…
Platform Additions and Upgrades
- Launched the Child & Youth dashboard in the Community Analytics Platform – Unlocking new data insights into pay for performance children’s health metrics via the Rural Well-Child KPI report.
- Enhanced the Community Analytic Platform’s Cancer Screening and Diagnosis report in July, with another update coming this month – be on the lookout for an official announcement coming soon
- Introduced Innovaccer to empower healthcare organizations with actionable insights into cost and utilization trends
- Enrolled 12 new participating facilities to the Rural Connectivity Program, increasing the number of total facilities to 117 and total attributed members in the CAP to over 700,000 – an increase of nearly 100,000 unique patients since December 2024
Expanded Technical Assistance & Shared Learning
- Delivered remote and on-site technical support and training on dashboard access, usage, and visual analytics
- Facilitated Super User Group and presented on Rural Connectivity Program’s impact on analytics and CRHC’s role in rural quality improvement
Quality Improvement Initiatives and Financial Impact
- Supported facilities projects through:
- Identifying new service lines
- Development and workflow optimization for cancer screenings and Chronic Care Management (CCM) programs
- Improve data collection practices to improve health equity
- Analyzing data for grant writing
- Provided Sustainability Payments to 111 eligible facilities. Each Critical Access Hospital and Rural Health Clinic who met the criteria received $100,000 and $20,000, respectively
- Released two Innovaccer Modules:
- Cost Management Module – includes trends on cost and utilization by inpatient, outpatient, medication, and professional services
- Risk Management Module – includes data on payer performance, gap to potential risk, and RAF scores
